Discharging Plan for Chronic Obstructive Pulmonary Disease Patients

The aim of this plan is to provide care according to the needs and requirements of the person who leaves the health institution, and should turn into a basic guide, specially for the assistance provided to people with chronic pathologies such as chronic obstructive lung disease that due to its complexity requires multiple care and in this way have an integral intervention that results in a better adaptation, detection of possible complications and a better life quality.
Being the daughter of a patient with chronic obstructive lung disease, and a nurse plusthe high incidence of this disease made me think about the elaboration of the leave plan. Therewere many needs and a lot of care required,and a lot of questions that arouse every single day from making sure that therewas agood communication, also a knowledge of the signs and symptoms of emergency that indicated an exarcerbation.The adequate use of medicine, the importance ofoxygen therapy in the long run,the supply of an appropriate diet based on the given recommendations, and a favorableenvironment taking into account his health condition so that an armonic, physical, family and spiritual environment could be achieved.
